Those are the factors that trigger a Will to Fight check. Every time one of those happens you need to make a check.
– warband reduced.. (your whole force)
– unit reduced.. (just that unit of troops)
– any of your units in line-of-sight of a fallen hero..

The number of men who flee depends on dice roll (ie. If you roll a 4 but need 6 then the test was failed by 2 and you lose 2 guys).
